The incidence of medication errors in neonatal intensive care unit is eight times higher than that of in adult intensive care units. Thus, improving neonatal medication safety is very important.The aims of the study were to 1) explore the learning experiences of the nurses in the neonatal intensive care unit on medication administration, 2) develop the Medication Administration Competence Inventory for Neonatal Intensive Care Unit nurses and perform psychometric tests, and 3) examined the relationships of demographic variables and the medication administration competence. A total of 17 neonatal ICU nurses were interviewed in the qualitative research. The methods of purposive sampling were adopted. Four main themes emerged by content analysis, including "understanding how to prepare small dose of the medication", "learning precautions during intravenous infusion of the medication "," seeking correct medication adminstration strategies"and" realizing the importance of following medication administration procedures and filing incident report" . Combining the results of qualitative research with literature and clinical experience,58 questions of "Medication Adminstration Competence Inventory for Neonatal Intensive Care Unit nurses " were preliminarily proposed. A total of 272 questionnaires by neonatal ICU nurses were collected for item analysis, exploratory factor analysis, confirmatory factor analysis and internal consistency analysis. Finally, a total of 21 questions, including three subscales, “practical ability”, “ability to apply knowledge,” and “vigilance ability” were validated through the second-order confirmatory factor analysis model. The Cronbach’s α of the entire scale was 0.914 with three subscales 0.846 (for practical executive ability), 0.79 (ability to apply knowledge), and 0.784 (for vigilance ability), while the correlation between various dimensions r = .593- .745. Medication Adminstration Competence Inventory for Neonatal Intensive Care Unit nurses (MACI-NICU) was further tested. A total of 305 nurses in the neonatal intensive care units from 16 hospitals participated. Stepwise multiple regression analysis was used to analyze the relationship between demographic variables and medication safety capability. The results indicated that the nursing seniority, the number of courses regarding neonatal medication taken in the past year, and the number of medication errors in the past year can explain the Variance of 16.7% of medication administration competence. The MACI-NICU is a reliable and valid clinical assessment tool, which can be used to evaluate the medication administration competence of nurses in the neonatal intensive care unit.
