Description
Summary:碩士 === 國立臺北護理健康大學 === 護理研究所 === 103 === ABSTRACT Hip fracture is one serious consequence of fall, and surgical treatment is one most common and effective management for hip fracture after falling down. Due to the long recovery period after surgery, high disability rate, in need of assistants from caregivers, medical and social costs; falling down, however, is an important and critical public health issue in aging society nowadays. Attention was paid to the treatment care plan and function recovery training at post-acute stage after receiving surgery among hip fracture elders. The purpose of this study was to explore the effectiveness of hip fracture elders participating in post-acute stage care plan after discharge after surgery, and the level of function recovery and depression. This was a cross-sectional research design with convenient sampling, with a total of 153 subjects recruited from orthopedic units at a northern Taipei medical center. Instruments included Chinese version of the Modified Barthel Index(MBI-C), Instrumental Activities of Daily Living(IADL) and Revised Geriatric Depression Scale- Short Form(Revised GDS-SF). Descriptive statistics, independent t-test, ANOVA, Chi-square test, Pearson correlation and linear regression were on board for statistical analysis. The results indicated that (1) there was no difference between participating in post-acute stage care plan and go home after hospitalization in daily activity function, instrumental daily activity function and depression; (2) the better the recovery in daily activity function , the better the instrumental daily activity function, and the lower the depression level; (3) the older the age and the more comobidity, the worse the daily activity function; (4) different recovery period demonstrated a significant difference on daily activity function. Therefore, in the clinical setting, recovery plan should be aggressively engaged for older and high comobidity patients to restore activity function. When implanting discharge planning service, rehabilitation facility with longer rehabilitation training program, or long term care center with long term home care service should be considered first. Recover to prior daily function status, elevation of life quality, decrease the burden of family and society can be then expected!
