The study used randomized control trial research design. Purposive sampling was used to select 80 outpatients of NYHA class I ~ Ⅱ stage heart failure in two medical center of northern Taiwan. The participants were randomly assigned to the intervention(n = 39) or control group(n = 41). The intervention group received a 12-week Baduanjin exercise program. It consisted of 35-minute Baduanjin exercise demonstration videotape, a picture-based educational brochure, a performance record form, and 2 week individual teaching course of the Baduanjin exercise. Then the participants were led by the demonstration videotape to perform home-based exercise program with 35 minutes, 3 times per week for 10 weeks (total 12weeks). The control group had no any intervention. Data were collected by a structural questionnaire including basic personal information, symptom distress scale, Pittsburgh Sleep Quality Scale(PSQI), the Hospital Anxiety and Depression Scale(HADS). Data were analyzed using statistical methods including percentage, mean, standard deviation, independent t test, repeated measures analysis of variance, and generalized estimating equations (GEE). The results show that 59.7% of heart failure patients have a poor quality of sleep, and the mean PSQI score was 7.42 (SD = 4.30). The most worst sleep quality are disturbed sleep, sleep latency, and subjective sleep. The mean anxiety score was 4.45 (SD = 3.55), and mean depression score was 5.73 (SD = 3.81). The intervention group after a 12-week Baduanjin exercise program had significantly improvement on sleep quality and psychological distress. The control group had worsened their sleep quality and psychological distress. In addition, compared to the control group, the intervention group had greater improvement on their sleep quality and psychological distress after 12 weeks exercise program. This study confirmed that the 12-week Baduanjin exercise program could improve sleep quality and psychological distress in patients with heart failure.
