Textbook selection has large repercussions for primary and secondary school education in Taiwan because it crucially influences teaching and learning activities. The textbook editions that are easy for teachers to use, enable them to teach effectively, and improve the learning outcomes and interests of students must be selected, and a comprehensive, systematic textbook selection index must be formulated. On the basis of the contents of the textbook evaluation indicators used for the Grade 1–9 Curriculum, we constructed five major dimensions and 16 evaluation indicators. In addition, we employed a fuzzy analytic hierarchy process to construct a textbook evaluation indicator-based assessment system for the Grade 1–9 Curriculum. After summarizing and organizing 12 experts’ questionnaire responses, we applied fuzzy theory to determine the relative weighting of various dimensions and indicators through specific and strict mathematical models. Integrated analysis and research were also performed. The results revealed that the critical dimensions in evaluating health education and physical education textbooks, in descending order of weighting, were learning content > teaching practice > curriculum goal > auxiliary measures > publication characteristics. Among the indicators under the dimensions, the highest weighting was allocated to providing students with creative thinking ability and further learning motivation. The second highest weighting was allocated to strengthening students’ ability to practice health behavior and exercise intelligence. The third highest weighting was allocated to a goal of balancing cognitive, affective, and psychomotor aspects. The result of this study revealed that FAHP could be used in building and constructing the weight value that evaluation alternatives for health and physical textbook objectively, it planned to be could build evaluation alternatives and weight value came out to construct support the grade teacher or health and physical textbook editor person of primary school as consulting, as the professional knowledge in improving the evaluation for health and physical textbook.
