It can be controlled effectively if people follow the sustained health care, improving the lifestyle and self-management skills. Therefore, there was a study aimed to evaluate the effectiveness of the health promotion program. A quasi-experimental study design was used. The study was conducted in 2015, from April to June in two elderly community care centers, one for experimental group (n=28), the other one for control group (n=27).Data were analyzed using SPSS version 22.0. Descriptive statistics, chi-square test, independent sample t test, Mann - Whitney U test were used. The results of this study showed: (1) Both systolic and diastolic blood pressure were reduced more in experimental group than control group (p=.002; p=.002);(2)The participants’ dietary behavior (p=.009), exercise behavior (p<0.01), and medication compliance behavior (p<0.01) has improved in the experimental group. According to the results of this study, health promotion program self-management intervention has significantly reduced blood pressure so that the health promotion program is feasible and effective. It can be a reference for designing elderly health promotion program to enhance people’s health. Keywords: health promotion program, community elderly, hypertension, self-management .
