Summary: | 碩士 === 輔仁大學 === 資訊管理學系碩士班 === 105 === Taiwan’s National Health Insurance model—in force since the mid-1990s—has clearly resulted in wasted medical resources, especially for the Hospital emergency department (ED). At present, the ED has become the most important and busiest unit within most hospitals, but also affect the health care of quality and obtain. Accordingly, ED overcrowding is a national problem that requires a promising approach for identifying solutions. For reducing the overcrowding problems in the ED, the Department of Health of Executive Yuan implemented and carried out five-level Taiwan Triage and Acuity Scale (TTAS) since 2010 to establish the priority of treatments for visits in the ED based on their severities of the diseases.
This research aims to investigating patients’ behavior in the ED under each triage. A decision tree method was adopted to predict and analyze patients' behaviors based on their individual information and the medical diagnosis information under different TTAS and Length of Stay (LOS). It also help us to examine the association between the ED overcrowding and TTAS from the perspectives of the patients' behaviors. Finally, we explain the patients' behaviors under different triage by information systems portfolio prioritization (ISPP) model. Futhermore, the cooperated doctor help us assess the clinic values of the decision tree and rules for explaining the patients’ behaviors in the ED. The major findings in this research are as follows: (1) The decision tree of the second level, i.e., emergency, of the triage has the highest clinical value and correctness; (2) Through the ISPP model, it can analyze and generalize the patient's behaviors after consulting with the attending physician; (3) Finally, we also confirmed the low-urgent patients with short LOS in the ED are the main cause of ED overcrowding. The research results provided valuable information about understanding the characteristics of patients under different triage levels in EDs and help the hospital to investigate their ED overcrowding problems.
