Patients with OHCA are mostly caused by heart attack. According to statistic data from Taiwan Ministry of Health and Welfare, heart attacks ranked the second out of ten certificated causes of death in Taiwan for the past decade (2004~2015). Early defibrillation is the key to the survival of OHCA patients. Hence, the Emergency Medical Act in Taiwan was amended in 2013 to include the regulation and legislation for implementation of public access defibrillation (PAD) for OHCA patients. In this study, we aimed to examine the coverage of OHCA patients in the current status of PAD setting in Tamsui District, New Taipei City, Taiwan. Geographic information system (GIS) technology was applied to compare the occurrence of OHCA patients (n=217) and locations of PADs (n=54). We also provided a simulation of new strategy that equip PADs in all 24-hour-open convenience stores (n= 102) in Tamsui District, and re-examine the changes of coverage in the simulation and the quantity of increasing need of PADs. After mapping by GIS, we found that the 5-min coverage rate of OHCA by PAD is 75% in the current setting, lower to a simulation condition of all PADs just being equipped no elsewhere but only in the convenience stores in Tamsui Dist (coverage rate 80%). Furthermore, the 5-min coverage rate will be upraised to 87% in the situation of current setting plus newly implanted PAD in all convenience stores in Tamsui Dist. Current PAD locations have a 5-min coverage of 75% for OHCA patients in Tamsui Dist. Equip PADs only in all convenience stores will reach the 5-min coverage rate of 80%, and extra setting in the convenience stores beyond current implanted locations in this area will upraise the coverage to 87% but with a need of near triple amount of PADs.
