Thus, to prevent patient suffer in fall or other injury relatively is the first goal we want to achieve in medical agencies. Objective: The aim of this study was to use the TW-FRHOP scale that is a multicomponent evaluated tool to establish an evidence study and to decline the opportunity of falls in patient. Method: This study is a non-randomized quasi-experiment, included the medical patient who aged above 50 lived in Tri-Service General hospital. Pre-test was practiced in 72 hours when patient admission and Post-test practiced in the day discharged, the TW-FRHOP total scores could be assembled 3 group, high risk(23-45), middle risk(15-22) and low risk(0-14), further, the TW-FRHOP strategies was a multicomponent falls prevention be used in patients during whom lived in medical ward. After that, the difference of fall risk factors between Pre-test and Post-test could be discussed how the efficiency of the TW-FRHOP intervention. Results: The mean aged was 65.6±11.3, mean inpatient days 7.59±3.15. Intervention group has six fall risk factors been proved after TW-FRHOP intervention including hearing, frequent toileting, food intake declined, weight loss, footwear and TW-FRHOP total scores. Control group has four fall risk factors been proved after observation including frequent toileting, food intake declined, footwear and TW-FRHOP total scores. Middle-high risk group in intervention has twelve fall risk factors been proved including falls history, falls injury, high risk medication, chronic disease, medication type, cognitive status, food intake declined, footwear, sensory function, balance, transfer and mobility, foot issue and TW-FRHOP total scores. Conclusion: In this study, so the low risk group patients in intervention group has few fall risk factors that could established a intervention strategies only for low risk patients after TW-FRHOP screen test that may do it in all low risk patients to decrease chance of fall, otherwise, middle-high risk patients be practice a customized intervention strategies, those strategies made from TW-FRHOP strategies. In the future, medical agencies to build a system of integrate multidisciplinary care team and standard operation process in prevention care of fall may have to decline the fall rate and decrease patients fell.
