The success of long-term care services depends on caregivers who directly take care disability person on first line. Therefore, the main caregivers physical and mental health are the keys of long-term care service system in Taiwan. When the main caregivers cannot take care of the task, it will affect the quality of long-term care services, and even cause personal physical, mental and family problems of the main caregivers, which will cause a new social problem. This study explores some environmental factors that will impact the major caregivers’ physical and mental health. A total of 496 useful data in Taipei city extracted from the long-term care classification system was applied for analysis. The relationships between the main caregiver stress, physical health, mental status, family interaction, self-conscious quality of life were discussed in this study. The dangerous groups of the major caregivers are the age distribution in the 51-60 and 61-70 years old, live together, 18-24 HR care, sleep without interference, physical load, and the behavior of the case is not troubled. That is, when the primary caregiver has a clear mind for a long time to take care of the caregiver, the major caregivers physical and mental health conditions are the most serious. Based on the analyzed results, we provided some policy suggestions for government to assist and improve the long-term care services.
