Although workplace bullying of nursing staff in the UK, Australia, Europe, and the United States is already an emphasized subject, it is found that the occurrence of workplace bullying in medical health care system is much higher than normal occupations. Objectives: This study mainly focuses on the explores the influence factors of work and organization to nursing staff being bullied. Further, to understand the impact on job performance. Methods: A qualitative interview was conducted and a total of 14 volunteers were enrolled. One-on-one questions were used to proceed semi-structured in-depth interviews. Interviews took about 30 to 90 minutes. After obtaining the approval of the IRB's review, the information was collected with prepared interview scripts, notes and simultaneous recording under interviewers’ agreement. In order to complement the deficiency information, when necessary, a telephone call revisit was made. After the interview, data were translated into a verbatim script and then coded, analyzed and summarized for interpretation. Results: The factors affecting the workplace bullying of nursing staff could be concluded to two major categories such as organization related problems and policy issues. Organizations related problems include the issue of the leadership style, unfriendly environment (interpersonal relationships), organizational fairness, improper organizational policies, etc. Policy issues include in-hospital and national medical care management policies. The bullying incident deeply affects the work efficiency and physical and mental health of nursing staff, eventually. Conclusion: Among the related factors of nursing workplace bullying, the job traits and job satisfaction of nursing staff are relatively equal. When job satisfaction is poor, it is prone to occurrence of workplace bullying. The leadership ability is also very important to first-line nursing employees. In fact, this can be considered to be the most important factor in the unit's bullying, resulting in poor work efficiency and performance, when the supervisor management is inappropriate. It is also found in the study that the latest and most specific issue has been identified as policy issues. Frequent national policy change without good corresponding measures could easily lead to a series of organizational workplace bullying. Suggestion: 1. It is recommended that hospitals have good open work practices and clear job assignments, open and transparent promotion system, and fair and systematic rewards and punishments regulations which must be implemented, truly. 2. Advocate a zero-tolerance policy for workplace bullying and anti-bullying actions. It should emphasize the two-way communication between the top and the bottom levels. The nursing supervisors must have good leadership skills, or set up an organization to fight against bullying. 3. It is essential to have a clear strategy before new policy implementation and a good supporting measures before national policy change announcement.
