Preliminary evidences indicate that the Type D personality is related to illness control and mental health of chronic diseases. Patients with type 2 diabetes also have higher perceived stresses and psychological distress. In the current study, for the first time that using the conceptual model of Cognitive Appraisal Process to explore the relationship among perceived stress, self-efficacy, self-management behaviors, and psychological distress in type 2 diabetes patients. Methods. 198 participants were recruited in public. Patients filled out DS14-TR to measure Type D personality, PAID-C to diabetes-related stresses, DMSES-C to the confidence of diabetes management, DSC-C to diabetes management situation, and HADS to their psychological distress. Results. There were significant differences in HbA1C and the habits of alcohol consumption between type D and non-Type D personality patients. Type D personality patients had higher level of diabetes-related stress and psychological distress, but lower in self-efficacy and self-management of diabetes. Self-efficacy and self-management behaviors played the roles as partial serial mediators between diabetes-related stress and psychological distress. Conclusions. The self-efficacy and self-care behaviors of illness management play the key roles in patients with Type D personality in type 2 diabetes treatments. Interventions aim to enhance the stress coping strategies and perception of self-efficacy, such as problem-solving skills training or cognitive reappraisal, could improve the ability of self-care and decrease the possibility of emotional disturbances.
