Though many drugs are used to treat the pain but the exact underlying mechanisms are remained unknown. On the other hand, osteoporosis is also a difficult problem with limited treatment efficacy. The causes and risk factors are still poor identified. Therefore, the present thesis report research on neurosurgery science through basic science and clinical practice. The report addresses the causes and risk factors of osteoporosis, the pain generating mechanisms and the relationship between pain and osteoporosis. Five studies were performed by using the nationwide population-based retrospective cohort study of data from the Taiwan National Health Insurance Research Database to clarify the osteoporosis risk factors and relationship between osteoporosis and pain risk. The association between peptic ulcer disease, erectile dysfunction and post-herpetic neuralgia and the risk of developing osteoporosis was estimated using a Cox proportional hazard regression model. The report also revealed that osteoporosis may induce subsequent migraine and trigeminal neuralgia. Two other animal studies were conducted which illustrated the effects of hepatoma-derived growth factor on pain relief, the anti-inflammation effects and anti-apoptosis effects of hepatoma-derived growth factor blockade treatment in resiniferatoxin -induced neuropathic pain model. The results showed blocking hepatoma-derived growth factor attenuated the mechanical threshold response, neuron cell apoptosis in dorsal root ganglion, and protein expressions of PI3K, TrkB, substance P及 p-Akt/Akt. Furthermore, the report showed that NGF-trkA signaling modulates the analgesic effects of prostatic acid phosphatase.
