The chronic diseases derived from Metabolic Syndrome, which rank as the top ten causes of death in Taiwan and have become an important public health issue in Taiwan. Traditional health education might cause information inconsistent and does not provide suitable health advices according to personal health needs, and it is difficult to repeat review and practice the health education. The developed App can help to improve the above shortcomings of traditional health educations. Moreover, the physiological record covered by the current Metabolic Syndrome Apps in the market are mainly focused on blood pressure, blood glucose, weight, waist circumference, exercise, walking, sleep, diet, and do not contain chronic-related personal medical information of the hospital. Therefore, the Health Education Management App, which is based on Metabolic Syndrome, has been set up to allow users to view the records and improve the physical condition to promote health through recommended teaching suggestions. Research Steps: first we interviewed the case manager to obtain information on the content of the health education and Metabolic Syndrome patients to obtain relevant needs, then create a Facebook community and develop an App, finally on-line test the developed App and collect feedback from patients after using the health education management App and make improvements. Research result: Patients with Metabolic Syndrome can use the Facebook community to obtain health education information and can also ask questions on-line. Through the App, patients with Metabolic Syndrome can see the fluctuation of their physical values at any time, evaluate their own health status through simple assessments, and use visualized interfaces to represent health analysis. To make patients with Metabolic Syndrome more aware of their own health conditions and can get appropriate health education recommendations through the App, regardless of time and place to learn the contents of health education.
