The evaluation form was issued under the Civil Service Performance Evaluation Act, but government entities were also allowed to revise their content of the form. However, most of them have not made any changes. The responsibilities of the health professionals at Public Health Clinics are different from those of general administrative staffs, yet they are reviewed by the same form. The purpose of this research is to examine whether the items of current evaluation form can be used to fairly evaluate the health professionals in Public Health Clinics. The research is conducted by interviewing ten people including four registered nurses, three medical technologists, and three directors with unstructured questionnaires. Teamwork, Customer Service, Process Simplification, Knowledge and Skills are considered most important, and language is considered to be a bonus item, while Modesty, Cautiousness, Hard Working and Annual Planning are considered unnecessary by the registered nurses. Job attitude and Customer Service are considered most important, while Annual Planning is considered unnecessary by the directors and the medical technologists. The registered nurses, the medical technologists, and the directors suggested that the proportions of criteria in the evaluation from should vary on the professional.
