It is mainly caused by relative lack of insulin in the body or insufficient sensitivity of tissue cells to insulin. The inability to effectively use carbohydrates causes abnormal metabolism of carbohydrates, fats and proteins, resulting in high sugar blood levels. Type 2 diabetes mellitus is mainly caused by insulin resistance. This type of patient is often accompanied by metabolic abnormalities such as obesity, hypertension, and hyperlipidemia. It is easy to produce excessive fat accumulation in the body and causes diabetic steatohepatitis, which is common in diabetes complication. If it continues to worsen, it will lead to liver fibrosis or cirrhosis, and the most serious may induce hepatocellular carcinoma. Therefore, prevention and treatment of diabetic steatohepatitis has become an important health issue in the world. According to our published literature, the mulberry leaf extract and mulberry leaf polyphenol extract can be used to ameliorate nonalcoholic fatty liver through the molecular mechanism of lipid synthesis and oxidation. In addition, the polyphenolic compounds contained in the mulberry leaf extract were isolated and identified, and it was found to be rich in three chlorogenic acid isomers, namely neochlorogenic acid (nCGA), cryptochlorogenic acid (Crp) and chlorogenic acid (CGA). However, the research mechanism of mulberry leaf extract and its main components for relieving diabetic steatohepatitis has not been clarified. Therefore, this study explores the relationship between mulberry leaf extract (MLE) and its main component chlorogenic acid isomers and diabetic steatohepatitis in a cell model. High glucose and high oleic acid induced HepG2 cell to produce fat accumulation, and MLE and its main components, chlorogenic acid isomers (nCGA, Crp, CGA) were added. The results showed that MLE and neochlorogenic acid (nCGA) can inhibit liver fat accumulation by activating AMPK, thereby inhibiting SREBP-1 / FASN and SREBP-2 / HMGCo-R and activating PPARα / CPT-1. In addition, MLE and nCGA can reduce inflammatory response by reducing inflammatory factors associated with NF-κB, TNF-α and IL-6 to alleviate diabetic steatohepatitis. Therefore, it is speculated that the neochlorogenic acid is the active ingredient in the mulberry leaf extract. Based on the above experimental results, this study confirmed that mulberry leaf extract and its main component neochlorogenic acid have the effect of alleviating diabetic steatohepatitis, and it is expected to be applied to the development of diabetes and liver-related health foods and natural medicines in the future.
