However, few researches focused on older adults and the association was not consistently. Therefore, the purpose of this study was to examine the association of the alpha actinin 3 R577X polymorphism on muscle mass, muscle strength and physical performance in older adults by meta-analysis. Methods: We conducted cross-sectional study and meta-analysis. Participants were older adults extracted from Health Management Center of Tri-Service General Hospital from 2016 to 2018. Demographic data were obtained by structured questionnaire. Muscle mass was measured by dual-energy X-ray absorptiometry (DEXA). We used Asian Working Group for Sarcopenia (AWGS) to determine the prevalence of sarcopenia in this study sample. T test, ANOVA and logistic regression were used to analysis the association of ACTN3 R577X polymorphism and sarcopenia. In meta-analysis, we searched relevant literatures from “PubMed” and “Embase” database (time to December 2016), and used TSA to verify results. Results: Older adults 277 men and 405 women (mean age: 73.34±7.02 years old) participated in this study. Although meta-analysis showed that ACTN3 R577X polymorphism wouldn’t influence decline in muscle mass, muscle strength and physical performance. We found that aging, physical activity modified the association of ACTN3 R577X polymorphism and Legs physical performance in women. A total of 9 articles were achieved by literature search. No association with ACTN3 R577X polymorphism muscular phenotypes were observed in older adults. In addition, The causal relationship can’t be confirmed by TSA because of insufficient sample size. Conclusion: No significant association was noted for muscular phenotypes in older adults. Thus, Our results suggest that the causal relationship can’t be confirmed by TSA because of insufficient sample size.
