Summary:碩士 === 國立雲林科技大學 === 工業工程與管理系 === 107 === Purpose: The prevalence of end-stage kidney disease in Taiwan is the highest in the world, the consumption of medical resources has become a health insurance financial a big burden.According to the report of 2015United States Renal Data System(USRDS), the incidence rate of ESRD is 458 people per million populations. And the prevalence rate is 3,138 people per million populations which is the highest one in the world. The government has been working on the prevention and treatment of chronic kidney disease since 2003 and requires patients to be regularly examined and returned, this study was designed to analyze the outcome of early intervention and regular follow-up in patients with chronic renal failure. Method: This study took a retrospective survey, collecting teaching hospitals in a certain area in the central region, participated in slow from 2006 to June 2018,999 patients with a kidney care network, according to the patient's basic data,returning situation, test report and disease course research.Statistical analysis methods include chi-square test, logistic regression and complex regression analysis. Result: The patient's regular return visit showed significant differences in the rate of change of glomerular filtration rate (GFR) and creatinine (Cr),the rate of GFR decline in regular return visits is slower than that of irregular return visits; The opposite of,the rate of increase in Cr of irregular return visits is higher than that of regular return visits. Expected benefits: If the program is subsidized, it can understand the effectiveness of the implementation of the chronic kidney disease care plan in the National Taiwan University Hospital Yunlin Branch to alleviate the deterioration of the disease, which can be used as a reference for future revision of the care model.
